My VA psychiatrist wrote in physician mental health notes related to Vietnam combate ptsd Axis V: 50/50. I understand that typically a gaf score is listed in this area. What does 50/50 mean? Thanks

Ask the doc on the next visit. It could represent 50 today and 50 for the year. Just a WAG.

He probably meant 50 current and 50 highest past year. With DSM5 it won't matter anyways. So don't stress about it.
